Resident Care Coordinator / Licensed Practical Nurse (LPN)

**Job Summary:**

Cabot Cove of Largo is the premier Five-Google Star 79-bed assisted living community. We are seeking a compassionate and organized Resident Care Coordinator/LPN to join our dedicated healthcare team. In this role, you will be responsible for coordinating and delivering high-quality care to our residents, ensuring their comfort, safety, and overall well-being. You will work collaboratively with interdisciplinary teams to develop and implement individualized care plans while providing nursing services according to the guidelines set forth by our facility and state regulations.

**Key Responsibilities:**

**Resident Care Coordination:**

  • Develop and implement resident care program that meets the needs of all residents within parameters of company policies and governmental regulations.
  • Assess residents’ and potential residents’ needs and preferences through observations, interviews, and health assessments.
  • Coordinate medication, hospitalization, physician visits and/or appointments, including transportation. Develop, implement, and monitor individualized care plans in collaboration with the healthcare team and family members.
  • Assist Executive Director with potential resident screening and required intake documents in accordance with company policies and procedures and governmental regulations. Coordinate new resident orientation process and assist with resident move-ins.
  • Manage and assist with daily living activities, promoting resident independence and dignity.
  • Ensure that all care plans are up-to-date and reflect any changes in the resident’s condition or preferences.

**Nursing Duties:**

  • Provide direct nursing care, including administering medications, wound care, and other treatments in accordance with physician orders.
  • Serve as a liaison between residents, families, and healthcare providers to promote clear communication and address any concerns.
  • Conduct regular meetings with staff to discuss resident needs, care plans, and any changes in health status or behavior.
  • Collaborate with physicians, social workers, dietary staff, and other healthcare professionals to ensure comprehensive care delivery.
  • Participate in case management meetings and quality improvement initiatives to enhance resident care and services.
  • Monitor resident health status and document vital signs, symptoms, and responses to interventions.
  • Supervise and perform, when required, medication administration.
  • Coordinate medication, hospitalization, physician visits and/or appointments, including transportation.

**Compliance and Documentation:**

Ensure compliance with state and federal regulations, facility policies, and best practices regarding resident care and documentation.

Maintain accurate and detailed documentation of resident care, assessments, and communications as required by regulations and facility policies.

**Team Collaboration:**

  • Support the recruitment and retention of qualified resident care staff.

  • Supervise orientation, training, and support for all resident care staff members in compliance with company policies and procedures and governmental regulations.
  • Develop nursing staff schedules to ensure staff resources are managed and scheduled efficiently to meet resident needs in accordance with budget.

**Qualifications:**

  • Current Licensure as a Licensed Practical Nurse (LPN) in Florida
  • Minimum of 2 years experience in a long-term care, rehabilitation, or similar healthcare setting preferred.
  • Strong knowledge of nursing principles, resident rights, and healthcare regulations.
  • Ability to effectively assess and respond to residents’ medical and emotional needs.
  • Excellent communication, organizational, and problem-solving skills.
  • Proficiency in electronic health records (EHR) required, experience with ALIS EHR preferred.
  • Proficiency with Microsoft Office Suite comfortable with personal computers.
  • Certification in CPR and First Aid.

**Physical Requirements:**

  • Willingness and ability to provide direct care of ADLs and IADLs as needed.
  • Ability to perform physical tasks including lifting, supporting, and moving residents.
  • Must be able to stand, walk, bend, and reach throughout the workday.
  • May require the ability to work in varying environmental conditions (including but not limited to noise, temperature, and exposure to biological hazards).
  • Fluency in written and spoken English with working knowledge of common geriatric medical terms.
